Subcontractor / Vendor Qualifications

Prime Contractors Inc. requests the following information for qualification purposes. All information provided will be kept confidential. We will not disclose your Personal or Company information to a third party without consent, unless we are required or authorized to do so by law or other regulation. Subcontractors/vendors should email the questions regarding Qualification to: estimating@primecontractorsinc.com

• Current IRS W-9 Form

• EMR Ratings for current and previous 2 years.

• Three (3) relevant Project Experiences  

• Company Certifications - HUB, MBE, WBE, etc. 

• Provide Company Safety Manual 

• Certificate of insurance issued to:

COI Must include:

•  General Liability - $1 million per occurrence/$2 million general aggregate

• Umbrella or Excess Liability - $5 million

• Auto Policy – $1 million, all owned and non-owned autos

• Workers Comp - $1 million

• Professional Liability for trades performing design/consultation - $1 million

• Pollution Liability for trades performing asbestos abatement or hot work - $1 million 

                 *Additional Insured endorsements required on all policies referenced above except workers comp.
                 *Waiver of Subrogation endorsements are required on all policies referenced above.​​

                 *If awarded a project - complete requirements will be described within the Subcontract Agreement section titled:

                   “CONTRACTOR’S INSURANCE REQUIREMENTS OF SUBCONTRACTOR.”
